Well, first you need to download it.... then get a stock (picture with no bg) or make a stock by rendering a pic, then take that pic, multiply it 10 or so times, cover your canvas with it, merge them all, go into filters, Motion blur, settings are 10 strength, create a new layer, fill it with a colour, then set the layer options to something transparent, put in another instance of the pic, above or below the overlay, then type a word...

viola, the most basic photoshop siggy ever.
